After scheduling your initial session, for your convenience, we have provided the following forms for download that may be required prior to your first meeting.
Fillable forms may require Acrobat Reader/Acrobat DC to fill them out online or on your computer. You will want to download the forms and open them using Acrobat Reader/Acrobat DC. If the form still opens in the browser window once you’ve installed Acrobat Reader, locate it on your computer, right click on the form and select “open with” and choose Acrobat Reader.
If you do not want to install Acrobat Reader, you can also fill the form out in your browser, and instead of “Saving”, choose Print and then select “Save as PDF” as your printer in the dropdown menu and click save.

For payment, we accept credit/debit only. For information on our current rates, please call our administrative office at 714-558-9266.
We do not currently accept insurance as a form of payment. However, depending on your health care plan, certain services may be reimbursed to you in full or in part by your health insurance provider. Some health care plans, like PPOs, offer reimbursement for treatment from out-of-network providers. This means that as the client, you would pay the full amount charged by your therapist and then submit a superbill (provided upon request) to your insurance for reimbursement. Other health care plans, like HMOs or Medi-Cal, do not reimburse treatment from out-of-network providers, which means that as the client, you would be responsible for the full amount charged by your therapist.
